MEMBERSHIP/PAYMENT APPLICATION
FAMILY ANNUAL MEMBERSHIP
The Family Annual Membership is designed for two or more family members to borrow up to six library items at a time. The parent or guardian is responsible for the loss, damage, or fines from borrowing materials with the library card. Members can borrow any library item they choose, but
they should choose age-appropriate materials.
$50.00 per year.
INDIVIDUAL ANNUAL MEMBERSHIP
Individual Annual Membership is designed for adults/teens to borrow up to four library items at a time. They are responsible for the loss, damage, or fines from borrowing materials with their library card. Members can borrow any library item they choose, but they should choose age-appropriate materials.
$40.00 per year.
STUDENT ANNUAL MEMBERSHIP
Student Annual Membership is designed for students under 30 years old to borrow up to three library items at a time. They may need/seek intellectual thought, documentaries, and non-fiction materials for research purposes. Also, they may need advice on how to cite sources, determine whether an internet source is reliable or just an opinion. They are responsible for the loss, damage, or fines from borrowing materials with their library card. Members can borrow any library item they choose, but they should choose age-appropriate materials.
$30.00 per year.
To join, please complete the Membership Application and include your membership type in the box. Leave the “library card no.” box blank. Email the form to: TheQuietHouseLibrary@gmail.com. Also, email proof of address such as a copy of a recent utility bill [received in the last 60 days], or any other mail which verifies your name and address.
If you do not feel comfortable submitting proof of residency, The Quiet House Library will mail a letter to your house, and you can return the postcard included to verify that you received it.
Borrowing library materials are done via the US mail, so verifying your address is vital. We ship the library materials to you in a box and you return them in the same box with a preprinted label.
Once the above steps are completed, you will be mailed a library card to begin borrowing library materials. You can also place items on hold.
Library Cards:
You are responsible for the loss, damage or fines incurred when borrowing materials with your library card, including your spouse/partner, and children.
Lost Cards:
You are responsible for all items borrowed on your library card. Please report a lost, missing, or stolen card to The Quiet House Library, immediately. Replacement cards cost $2.00 each.

If you prefer to pay the annual membership fee via check or money order you can mail your fee along with the membership application and proof of residency to: The Quiet House Library, 5011 Sansom St., #9379, Philadelphia, PA 19139. Be sure to include your state and local sales tax in your total membership fee.
Afterwards, email The Quiet House Library, so that we can locate your order, process your membership and mail your library card.
